Extremes in hyperparathyroidism: management of parathyroid carcinoma

Radomir Lidia , Mihai Daniel , Jercalau Simona , Badiu Corin

Background: Parathyroid carcinoma (PC) is a rare cause of primary hyperparathyroidism (PHPT), accounting for <1%, with an equal gender distribution and an average age of diagnosis in the fifth decade of life. The diagnosis of PHPT is based on the laboratory finding of high levels of immunoreactive PTH in the presence of severe hypercalcaemia. The only potentially curative treatment for PC is surgery. Early surgery is the most important factor for optimal outcome.<p cla...

Virilisation due to a Leydig cell tumour of the ovary: diagnostic and therapeutic challenges

Dobrescu Ruxandra , Poalelungi Cristian , Ceausu Iuliana , Hortopan Dan , Badiu Corin

Introduction: Severe hirsutism and virilisation, especially if occurring later in life and with rapid onset, should prompt the search for rare but potentially threatening causes such as an androgen secreting ovarian or adrenal tumour. We present the case of a 47-year-old woman referred to our clinic with obesity, severe hirsutism, alopecia, acne, and deepening of the voice that appeared insidiously 2 years ago, along with amenorrhoea which she interpreted...

Challenges in exophthalmia management: Frontal mucocele

Toma Nicoleta , Mocanu Bogdan , Daniela Alexandrescu , Badiu Corin

Frequently seen in connection with thyroid disease, exophtalmic syndrome has a much large spectrum of differential diagnosis including numerous local and general aetiologies. Conditions such as mucoceles of paranasal sinuses don’t have always nasal or sinus symptoms but may present with ophthalmological manifestations.A 50-year-old woman presented for a 2-months history of right eye proptosis with upper eyelid swelling, periorbital pain, occasionall...

Small papillary thyroid carcinoma: a ‘benign’ condition?

Dobrescu Ruxandra , Ioachim Dumitru , Simioniuc Anca , Badiu Corin , Goldstein Andrei

Often discovered incidentally, small foci (≤2 cm) of papillary thyroid carcinomas (PTC) are its most frequent presentation. Despite favourable prognosis, the extent of surgery and use of radioablation is still controversial. Objective: Prospective cohort study of all patients admitted between 2007 and 2011, for thyroid surgery in the Institute of Endocrinology, Bucharest, Romania, and had pT1a or pT1b PTC.Patients and methods:...

Management of adrenal carcinoma in a tertiary center of Endocrinology in Romania

Jercalau Simona , Stanescu Bogdan , Tomulescu Victor , Caragheorgheopol Andra , Badiu Corin

Adrenal cortical carcinoma is a rare disease with an incidence of 1–2/million per year. Diagnosis is related to autonomy of adrenal steroid synthesis, tumour size above 6 cm, local invasion and tendency to recurrence. Treatment consists in adrenal surgery, non-specific chemotherapy and radiotherapy, and, more specific-Mitotane (Lysodren).Pathology diagnosis is based on capsular and vascular invasion, histological changes suggestive for malignancy an...

Adults with GH deficiency have subclinical longitudinal left ventricular dysfunction, without significant vascular function impairment, suggesting intrinsic myocardial disease

Badiu Corin , Mihaila Sorina , Mincu Raluca , Dobrescu Ruxandra , Vinereanu Dragos

Aims: GH deficiency (GHD) is associated with increased cardiovascular events. We aimed to evaluate cardiac, arterial, and endothelial function, by conventional and 2D speckle tracking echocardiography (STE), and biomarkers (proBNP and troponin I), in GHD patients by comparison with normal individuals with similar cardiovascular risk profile. The study included 52 GHD patients (46.9±15.6, 36 males), free of cardiovascular disease, severe hypertension ...

Measurement of hGH in acromegalic patients under pegvisomant treatment

Purice Mariana , Gheorghiu Monica , Jercalau Simona , Badiu Corin

Generously supported by IPSEN)-->Treatment with hormone analogues can challenge the precise measurement by interference with the routinely used immunoassays systems, related to the presence of autoantibodies in the serum, interference with associated therapy or the presence of altered forms of the hormone, with modified biological activity and/or immunoreactivity. In patients with acromegaly treated with the hGH...

Medullary thyroid carcinoma – a narrow surgical window

Dobrescu Ruxandra , Iacob Diana , Stanescu Bogdan , Badiu Corin

Introduction: Occurring sporadically or as part of a familial syndrome, medullary thyroid carcinoma (MTC) is insidious in onset, aggressive in behavior and invariably a challenge to the clinician and surgeon. Objective: To define the relationship between biochemical parameters, disease progression, and treatment outcome in patients with MTC.Patient and methods: We studied 35 patients (11 men, 24 women) aged 50.9±14.8 years, adm...

Between genetics, guidelines and treatment in MEN2A: a family affair

Dobrescu Ruxandra , Hortopan Dan , Stanescu Bogdan , Radian Serban , Badiu Corin

The multiple endocrine neoplasia syndrome type 2A (MEN 2A) is the consequence of a heritable mutation in the RET proto-oncogene, leading to a very high predisposition to develop medullary thyroid carcinoma (MTC), pheochromocytoma and primary parathyroid hyperplasia. Screening of ‘at-risk’ family members and prophylactic thyroidectomy is recommended – but not always easily accomplished. Our proband was diagnosed with MTC in 1977 (at age 21),...

Functional evaluation of the gonadal axis - is it useful for the etiological diagnosis of hypogonadotropic hypogonadism?

Dobrescu Ruxandra , Jercalau Simona , Caragheorgheopol Andra , Badiu Corin

Objective: To study the role of the GnRH analogue Triptorelin in the assessment of the hypothalamic-pituitary-gonadal axis, in guiding therapy for infertility and identifying hypogonadic patients with recovery potential (functional hypothalamic dysfunction and idiopathic hypogonadotropic hypogonadism). Methods: Dynamic testing using subcutaneous injection of 100 μg Triptorelin was performed in 29 adults (11 women) with hypogonadotropic hypogonadism (...
